Families facing trauma, housing instability, violence, and economic stress often encounter long waitlists, transportation barriers, and high costs when trying to access mental health care. The FACTS Initiative’s Community Partnership Program helps remove those barriers by embedding services within trusted local nonprofits, so families can receive support in places they already know and trust.

By providing care onsite and through flexible telehealth when needed, FACTS makes it easier for children, youth, and caregivers to begin care sooner, stay engaged, and build greater stability over time.

 

What our Community Partnership Program provides:

  • We provide trauma-informed services at partner locations where families already receive housing, food, education, and other supports.

  • When onsite services are not the best fit, secure telehealth options help families access care with greater privacy and flexibility.

  • Services include support that helps clients understand stress and trauma, strengthen coping skills, regulate emotions, and build resilience.

  • We work closely with community partners through warm handoffs and ongoing communication so care is connected to each family’s broader goals and support system.

 
 

Why This Model Matters

Traditional systems often ask families in crisis to navigate multiple referrals, paperwork, transportation challenges, and long wait times before they can even begin care. For many families, those barriers mean therapy never starts or ends too soon. FACTS removes those hurdles by integrating mental health support into trusted community settings.

This approach improves follow-through, aligns care with families’ immediate needs, and helps emotional healing connect to real-life stability in housing, education, food security, and daily functioning.
